Change of Visa Status (COS) Request


Persons holding a non-immigrant visa status in the United States may extend or change their visa status with certain limitations while remaining in the country. In order to request a change of visa status (for example from F-2 to F-1 or from H-1 to F-1), the following information is required from the requester:

o Completed I-539 form (INS form for requests to extend or change visa status without leaving the US)

o Check for $120 made payable to INS (Immigration and Naturalization Service)

o Copy of passport demographic page and visa page (this is to establish identity and legal status in the U.S.)

o Original I-94 card

o Affidavit of support, bank statement, assistantship agreement or other proof of sufficient funding

o Original I-20/IAP-66 or I-129 as appropriate, issued by the Graduate School

If the change of status request is based on a spouse's visa status, the following information is required in addition to the information listed above:

o Copy of spouse's passport (demographic and visa pages)

o Copy of spouse's I-94 (front and back)

o Copy of spouse's I-20/IAP-66 or other relevant immigration documents

o Affidavit of support, bank statement, assistantship agreement or other proof of sufficient funding showing that both the requester and spouse have sufficient funds.
